PAYMENT POLICY:

We are very sensitive to the cost of medical care. We strive to contain costs where we can by keeping your health, avoiding costly Emergency Room visits, using generic medication when possible, and limiting expensive testing and referrals to when we feel it is truly necessary. You will receive another handout on office policies and procedures for your signature. Copays are due before appointments. If we are not filing with insurance payment is due at the time of service. We do honor a 10% discount to patients with no insurance when paying in full. We strive to have all charges ready at check out time . If  charges are missed (ie labs) a statement will be sent to you with appropriate discount. It is your responsibility to notify us of any changes in your insurance coverage. It is your responsibility to bring your current card to each visit. If we do not have your insurance card within 3-business days of your appointment, the cost of your visit will be your responsibility.

We recognize how important it is in a doctor – patient relationship that you feel comfortable communicating with us freely and honestly about your medical needs. We hope that in selecting a doctor at SAMC you are able to find a medical home where you are treated with dignity and respect and where a dialogue about your health can be carried on in trust.

Health Insurance Terms

Workmen’s Compensation: We do NOT participate in this insurance plan or manage on the job injuries. However we can only see you for the initial visit.  

Third Party Payers: Our office does NOT bill third party payers such as Personal Injuries Protection for a motor vehicle accident, or attorneys. We do not file tertiary insurance.  

We accept Cash,Check, Visa, MasterCard and Debit cards for payment.
