Immunization Requirements in Texas Schools

As the school year ends most of our children will need an Athletic, Camps & Sports Physical for the next school year. Be prepare and avoid long wait times, call ahead for an appointment. 

Patients must bring health insurance information and an updated shot record in order to be seen. Patients must be accompanied by a parent or legal guardian. 
  
SAMC accepts most insurance plans and offers discounts to cash-paying patients at the time of the visit.
